All Quality Doors orders are custom made and payment is requested via VISA, Mastercard, American Express, check, or money order in advance. Please be sure to read the ordering policy. It is important that your measurements are correct. Once an order is placed into production, it cannot be changed.
If you wish to print this form and mail or fax it, that is okay. If you mail or fax it, you can write your credit card number and expiration date in the comments field if it is a firm order. If just a quotation, do
not give us any credit card info at this time. Fax transmissions come directly to Eclectic-ware. No one else will see your confidential information. Credit card information is never forwarded to Quality Doors. It is kept
